Join the NSCI Youth Group for a day of service honoring the memory of Dr. Martin Luther King, Jr. Please bring donations for Cradles to Crayons to help children in need. We will collect and sort donations, then head out for lunch and fun at Dave & Busters. Come and help us make a real difference! This program is for 3rd – 6th graders.

DETAILS

1. Please bring donations for our Cradles to Crayons Drive using this list for what they collect and the things they need most. Please consider letting people know you are holding your own collection—perhaps they could even drop items at your house that you could bring to NSCI.

2. We will meet at NSCI at 10:45 am to learn about Cradles to Crayons, help sort and prepare donations, and create well-wish notes. We will be having donuts as a snack.

3. Afterward, we will head to Dave & Buster's for a late lunch and lots of fun! (Price includes round-trip transporation, buffet lunch, a Power Card for ticket-generating games, and unlimited video games.)

FINE PRINT

Your registration form includes the required permission acknowledgment for travel by bus and emergencies. There is no need to bring additional money. Please contact Andrea at least a week prior to the event if your child has any food allergies or restrictions. Space is limited—please respond ASAP to hold your spot!

Cradles to Crayons provides children from birth through age 12, living in homeless or low-income situations, with the essential items they need to thrive—at home, at school, and at play. They supply these items free of charge by engaging and connecting communities that have with communities that need. All our donations will stay in the Chicago-land area.

Confidential scholarship assistance is available for all youth group events. Contact Andrea Dalinka, andread@nsci.org for assistance.

Parent's Consent for Participation & Emergency Medical TreatmentI hereby give permission for my child to participate in the youth group event on Monday, January 20, 2025. I do hereby authorize the Youth Director or agents of North Shore Congregation Israel to act as our agents to consent to any medical treatment deemed advisable by a duly licensed physician, in the event such help of an emergency nature becomes necessary.

Parent's Consent for Travel by BusI hereby give permission for my child to travel by bus during the youth group event on Monday, January 20, 2025. I release North Shore Congregation Israel and all their agents, officers, clergy, and employees from any liability whatsoever, including personal injury. I am aware my child will be traveling by bus between the synagogue and Dave & Busters in Vernon Hills.

Photos and Videos

From time to time, we take pictures or make videos of our programs. These images are used to decorate our walls, publicize programs, and further Jewish living and learning. We may use the images in print, on our web site, or on social media. Children’s names are not publicized with the pictures or videos. By submitting this registration, you agree to allow us to use pictures and videos of your child/children and/or your family in any medium for educational, promotional, or other purposes that support the mission of North Shore Congregation Israel.
